Most often, it happens with gals under thirty or gals new to the biz, or gals from CL or BP. This is precisely why I never see gals from CL or BP, seldom under thirty, or seldom before they've been around long enough for the 'new' to wear off, i.e. no Flavor (or flake) of the month... If the words 'up to an hour' ever appear anywhere in her ad, she's permanently persona non grata for me. You can't say "up to an hour" and say "GFE" in the same place. It's a different mindset. It's a habit she'll never break. I research exhaustively to cut down on the likelihood of it happening to start with.
